Background: Malnutrition and surgical stress impair recovery after colorectal resection. Objective: To synthesise evidence on oral
nutritional supplementation (ONS) and its integration with Enhanced Recovery After Surgery (ERAS) pathways. Methods:
Narrative review of published randomised trials, systematic reviews and meta-analyses, and mechanistic studies on perioperative
nutrition in colorectal and gastrointestinal surgery. Findings: Perioperative ONS, particularly immunonutrient-enriched
formulas, is associated with fewer infectious complications and shorter length of stay, especially in malnourished or at-risk
patients, though effects on well-nourished patients are more modest. Conclusions: Routine nutritional screening and ONS should
be embedded within ERAS pathways for elective colorectal surgery.
