Background: Chronic shunt-dependent hydrocephalus complicates a substantial minority of aneurysmal subarachnoid
haemorrhage (aSAH) survivors. Early identification could guide external ventricular drain (EVD) weaning and shunt timing.
Objective: To review published machine learning (ML) models predicting shunt dependency after aSAH alongside the clinical
scores and EVD-weaning strategies they are benchmarked against. Methods: Narrative review of derivation and validation studies, systematic reviews and meta-analyses, and EVD-management cohorts identified through PubMed, Crossref and journal searches.
Findings: Traditional scores such as CHESS and SDASH achieve areas under the curve (AUC) of roughly 0.75-0.82. ML models,
including random forest and gradient-boosting classifiers, report AUCs of 0.80-0.92 in derivation cohorts, with pooled
meta-analytic estimates near 0.79-0.80; one dual-centre external validation retained an AUC of 0.867. Most studies remain
single-centre with heterogeneous predictors. Conclusions: ML models modestly outperform traditional scores but need larger, externally validated, prospectively reported cohorts before routine adoption alongside standardised EVD-weaning protocols.
