Background: The hydrodynamic theory attributes dentin hypersensitivity pain to fluid movement within open dentinal tubules, linking the symptom to a structure histology and scanning electron microscopy (SEM) can directly visualize [2,8].
Objective: This review evaluates whether histopathological analysis of dentin and the pulp-dentin complex functions as a reliable prognostic tool, predicting symptom persistence, treatment response, and the limits of dentin-directed therapy.
Methods: We synthesized SEM and histologic studies comparing sensitive and non-sensitive dentin, treatment-response studies confirming tubule occlusion, the tertiary-dentinogenesis literature, and clinical-histologic correlation studies of the pulp-dentin complex.
Findings: Sensitive dentin shows markedly more numerous, wider open tubules than non-sensitive dentin [8], and treatment response tracks histologically confirmed tubule occlusion while non-response tracks persistent patency [11]; reactionary dentinogenesis offers a plausible but less directly tested prognostic axis [15], and clinical-histologic correlation in deep caries shows only moderate agreement with underlying histology [18].
Conclusions: Histopathological analysis, principally SEM assessment of tubule occlusion, carries genuine prognostic value and is best used as a bench-validation tool for interpreting treatment response rather than a chairside diagnostic test [8,11].
