Background: Dry eye disease prevalence rises with age to roughly 15% in the oldest cohorts, coexisting with the posterior segment diseases driving most irreversible vision loss [1,12].
Objective: To examine dry eye management in elderly patients and how fundus photography and surface imaging support efficient assessment.
Methods: Narrative synthesis of TFOS DEWS II reports, Cochrane reviews, cohorts, and fundus-photography screening literature.
Findings: Meibography documents evaporative dry eye [8], and non-mydriatic fundus photography now screens for retinopathy in the same visit [29,31].
Conclusions: Visual complaints should not be attributed to the ocular surface without a fundus photograph confirming posterior segment health [10,11].
